A LIC policy (no. R 1380015) issued on 13 July 19 … was held by my husband, Shri Suresh Bajpai, Sectional Officer, Catering Services, Northern Railway, Baroda House, New Delhi. On 7 October, 19… he died of a road accident in Delhi. An FIR was lodged with the Daryaganj Police Station.
Kindly send me the requisite application form so that I may apply for the risk/accident covered claim since I am the nominee for the same policy. Also kindly expedite investigation etc. so that the claim may be approved quickly since I am not an earning member and have two children to support.
The necessary documents shall be sent to you whenever desired.
